Flu and COVID-19 Prevention Efforts

Company Update – May 20th 4:00PM

Dear Residents, Families, and Associates:

During the past two weeks we have had several new cases of COVID-19 at several of our communities. In these cases, community specific letters have been distributed with the applicable information. Based on today’s information, we will have two of our thirty-three communities in Red Phase next week. Our company COVID leadership team has been carefully reviewing the trends in both positive cases and the impacts on our residents and associates. As a reminder, we will begin providing updates on a bi-weekly basis, as opposed to weekly, moving forward. The next companywide letter will be on Friday, June 3rd, 2022. Individual communities will continue to provide local updates if there are any cases of COVID-19 and/or if there are any changes in community Phasing.

Since the beginning of the pandemic we have been focused on following national, state, and local guidelines, while also trying to balance safety/infection control along with the impact of isolation on our residents. We continue to ask that all residents, families, and associates increase their diligence at following the best practice protocols to prevent the spread of COVID and other illnesses. Practice good hand hygiene, wear masks at all times in the community, and don’t enter the community, or interact with others if you don’t feel well. There have been some questions come up about the updated CDC indoor mask guidance. So far, the guidance has been relative to general settings, not healthcare relates settings, so our prior protocols remain in place.

All current CSL associates are required to be fully vaccinated unless they have an approved exemption, effective 2/1/2022 new hire associates must have at least their first vaccine dose, or an approved exemption, in advance of their start date, and agree to obtain the second dose (if applicable) within 30 days. We certainly hope that any residents and/or families who have been uncertain, and chose not to receive the vaccine initially, will reconsider and work with our community teams to determine the best way to access the COVID-19 vaccine.

We encourage all residents, associates and families to also receive the COVID Booster shots. Please reach out to your Executive Director or Resident Care Director if you have any questions. There have also been some questions about a second booster shot. A second booster shot has now been approved for those over 50 years of age. Our community teams will be working to make these available to residents and associates who are eligible, if desired.

While we will continue to follow all of our infection control protocols, we do anticipate the likelihood of having some additional cases of COVID-19 that will occur, especially with the new Omicron variant and potential for future variants. Effective August 9th 2021, all nonvaccinated
associates, private duty aides, etc are required to undergo weekly COVID-19 testing.

Our current phased reopening plan is a three phase plan, Green, Yellow, and Red and it was updated this week. Green Phase will be for all communities that have not had three or more resident or associate cases of COVID-19 in the past 10 days, Yellow Phase will be for
communities that have had three or more cases of positive associates and/or residents within ten days. Moving forward, Red Phase will only be implemented as directed by the Department of Health or other agency, or by the CSL COVID Response Team. We certainly hope that we can avoid and/or limit the Red Phase situations as much as possible. Red Phase does allow for scheduled visitation, in designated areas, for all residents who are not actively quarantining, either for a positive COVID diagnosis or for exposure.

The new schedule for communities “Re-Opening” is below. Please understand that this is the current projection, and it was developed based on several factors, including assumptions of pending tests. Changes in those assumptions and/or local/state guidance could impact/change
these dates. Federal, State, and Local Health Department guidance will always supersede our internal planning document.

For the Week of May 23rd, 2022, all CSL communities except for the following are scheduled to be in Phase Green.

CSL at Gloucester House- Red Phase
CSL at Radford- Red Phase

If you have specific questions, please reach out to your individual community Executive Director and/or leadership teams. If you have additional questions, we have a toll free CSL hotline. Messages left on this line will be responded to the next business day by an appropriate

CSL continues to encourage all residents to receive the COVID-19 vaccine and booster. We learned this week that Moderna has sought FDA authorization for a booster for its single dose vaccine. Company Update – February 18th 4:00PM

Dear Residents, Families, and Associates:

This past week we have continued to have a few additional cases of COVID-19, but thankfully the numbers continue to decrease. Based on today’s information, we will only have five of our thirty-four communities in Red or Yellow Phase next week. Our company COVID leadership team has been carefully reviewing the trends in both positive cases and the impacts on our residents and associates. We are pleased that in the majority of circumstances, those who are fully vaccinated are having milder symptoms and requiring significantly less instances of hospitalization.

Since the beginning of the pandemic, we have been focused on following national, state, and local guidelines, while also trying to balance safety/infection control along with the impact of isolation on our residents. Based on the trends that we have been experiencing, and other national information that we have gathered, we made a significant change to our Phased Reopening policies effective 1/21/2022. We now move into Red Phase when we have three or more residents who test positive for COVID-19 within a ten-day period. Similarly, we now move into Yellow Phase when we have three or more associates test positive for COVID-19 within a ten-day period. If there are less than three
residents, or three associates who test positive within a ten-day period, communities will continue to operate in Green Phase.

We continue to ask that all residents, families, and associates increase their diligence at following the best practice protocols to prevent the spread of COVID and other illnesses. Practice good hand hygiene, wear masks at all times in the community, and don’t enter the community, or interact with others if you don’t feel well.

CSL continues to encourage all residents to receive the COVID-19 vaccine and booster.

While we will continue to follow all of our infection control protocols, we do anticipate the likelihood of having some additional cases of COVID-19 that will occur, especially with the new Omicron variant and potential for future variants. Effective August 9th, 2021, all non-vaccinated associates, private duty aides, etc were required to undergo weekly COVID-19 testing.

Our current phased reopening plan is a three-phase plan, Green, Yellow, and Red. Green Phase will be for all communities that have not had three or more resident or associate cases of COVID-19 in the past 10 days, Yellow Phase will be for communities that have had three or more cases with positive associates, but not residents within ten days. Red Phase will apply any time there are three or more positive resident cases within a ten-day period, in the community. We certainly hope that we can avoid and/or limit the Red Phase situations as much as possible. Red Phase does allow for scheduled visitation, in designated areas, for all residents who are not actively quarantining, either
for a positive COVID diagnosis or for exposure.

The new schedule for communities “Re-Opening” is below. Please understand that this is the current projection, and it was developed based on several factors, including assumptions of pending tests. Changes in those assumptions and/or local/state guidance could impact/change these dates. Federal, State, and Local Health Department guidance will always supersede our internal planning document.

For the Week of February 21, 2022, all CSL communities are scheduled to be in Phase Green, except for the following:
CSL at Cedar Bluff- Red Phase
CSL at Hillsville – Red Phase
CSL at Kilmarnock – Red Phase*
CSL at Radford – Red Phase
CSL at Oak Ridge- Yellow Phase**

*CSL at Kilmarnock – Anticipated return to Green Phase 2/20
**CSL at Oak Ridge – Anticipated return to Green Phase 2/25

If you have specific questions, please reach out to your individual community Executive Director and/or leadership teams. If you have additional questions, we have reestablished a 24/7 toll free CSL hotline that is manned by members of our home office and regional support teams. That number is (866) 504-1053.
